Care Finder
As a Care Finder organisation, Hutt St Centre links people to practical assistance for vulnerable older individuals who are unable to independently arrange the services they need and lack support from family or friends.
Services offered through Care Finder include:
- Personalised assistance: One-on-one meetings to understand individual situations and develop tailored support plans.
- Aged care navigation: Guidance through the process of accessing aged care services, including communication with My Aged Care, arranging assessments, and selecting providers.
- Form completion: Assistance with paperwork and understanding service agreements.
- Ongoing support: Regular check-ins to ensure services are meeting needs and to address any challenges.
- Community connections: Linking individuals to additional supports such as health services, housing assistance, and community groups.
Accessing the service
To be eligible for Care Finder, individuals must be:
- aged 65 years or older.
- If someone is experiencing homelessness, demonstrating premature aging with associated frailty, they may be able to access My Aged Care Services earlier (from 50).
- prematurely aged with associated frailty.
- lacking a carer or support person, or not having someone they feel comfortable or trust to support them.
- eligible for government-funded aged care.
- facing challenges such as communication difficulties, reluctance to engage with services, or unsafe situations without support.
- reside within the City of Adelaide, Town of Walkerville and the City of Norward, Payneham and St Peters.
